根據《預防及控制疾病（疫苗通行證）規例》（第599L章）及《預防及控制疾病（規定及指示）（業務及處所）規例》（第599F章）下的最新指示及指明下的規定及限制，所有進入或身處指明處所的人士， 均須符合規例所列的新冠疫苗接種要求，獲豁免情況除外。

由2022年2月24日起，請各服務使用者根據以下指引，在進入本會所前進行相關登記。多謝合作！

2. 利用手機「安心出行」應用程式掃描場所二維碼

15 歲或以下﹑65歲或以上或殘疾人士而未能使用安心出行的人士：使用本會所提供的訪客登記表格登記姓名、聯絡電話及到訪處所的日期及時間（資料將保留記錄31天）。

15 歲或以下，在成年同行人士陪同下而未能使用安心出行的人士：若其成年同行人士已使用安心出行掃描場所二維碼，則相關人士無須以指定表格登記有關資料。

3. 出示相關「疫苗通行證」(12歲以下的兒童可獲豁免)：

a) 以「安心出行」、「智方便」或「醫健通」流動應用程式顯示的疫苗 接種紀錄或「豁免接種疫苗證明書」二維碼；

或

b) 紙本疫苗接種記錄上的二維碼或「豁免接種疫苗證明書」由會所職員使用「驗證二維碼掃瞄器」流動應用程式掃瞄相關二維碼並保存 31天的紀錄。

According to the Prevention and Control of Disease (Vaccine Pass) Regulation (Cap. 599L)  and the Prevention and Control of Disease (Requirements and Directions) (Business and Premises) Regulation (Cap.599F), all persons entering or remaining on specified premises must comply with the COVID-19 vaccination requirements as set out in the regulation, except under exempted scenarios.

Starting from 24 February 2022, all users must first register before entering the premises. Please see details below. Thank you for your cooperation.

2. Scan the "LeaveHomeSafe" (LHS) venue QR code using the LHS mobile application

Users aged 15 or below or aged 65 or above or persons with disability who are unable to use the LHS app: Use the specified form as provided by the Centre to register their names, contact numbers and the dates and times of their visits. The records will be kept for 31 days.

Users aged 15 or below who are unable to use the LHS app and are accompanied by an adult: If the accompanying adult has already scanned the LHS venue QR code using LHS mobile application, the relevant users do not need to register the relevant information using the specified form.

3. Show the relevant "Vaccine Pass" (children aged under 12 are exempted): 

a) QR code of vaccination records or COVID-19 Vaccination Medical  Exemption Certificates saved in the “LeaveHomeSafe”, “iAM Smart” or“eHealth” mobile apps;

or 

b) Paper vaccination records or COVID-19 Vaccination Medical Exemption Certificates. Our staff will scan the relevant QR code by using the “QR Code Verification Scanner” mobile application and keep the log record for 31 days.
